A 1-page sheet to help you choose the most important things to tell your doctor, and help them understand what’s going on.

Take a screenshot. Open it on your phone at the doctor’s visit, and use it to ping your memory of things to talk about.

Start one week before your appointment. Fill it out. Have someone close to you review it and add things. Send a copy to your doctor via email before the appointment. Print a copy to bring with you too.
